In consideration of participating in firearm related activities with the “Annie Oakley Shooters of Illinois” or “AOSNI”, the participant agrees to the following:
For the purposes of this document, “Annie Oakley Shooters of Northern Illinois” shall mean any and all members of the AOSNI and any of its board of directors, employees, representatives, agents, independent contractors, or volunteers (including instructors, organizers, promoters, et al) of such.
Participant agrees to indemnify, hold harmless and defend the AOSNI from all injuries, fault, liabilities, costs, expenses, damages, or lawsuits arising out of, related to or connected with: Participant; Participant’s presence or use of the range, building, vehicles, land and premises; and any and all acts or omissions of Participant or the AOSNI.
Participant furthermore waives for herself/himself and for her/his executors, personal representatives, administrators, assignees, heirs and any next of kin; any and all rights and claims for damages, losses, demands and any other actions or claims whatsoever, which she/he may have or which may arise against AOSNI (including but not limited to the death of the Participants and/or any and all injuries, damages or illness suffered by Participant or Participant’s property), which may in any way whatsoever arise out of, be related to or be connected with the participation or presence by Participant; the premises, including any latent defect in the premises; Participant’s presence on (or off) or use of said premises; Participant’s property (whether or not entrusted to AOSNI); and the discharge of firearms. AOSNI shall not be liable for, and Participant, on behalf of herself/himself and on behalf of her/his executors personal representatives administrators, assignees, heirs, and next of kin, hereby expressly releases the AOSNI from any and all such claims and liabilities.
Participant hereby expressly assumes the risk of taking part in instruction of firearms and taking part in the activities on the premises, which include , but are not limited to , instructions in the use of firearms, the discharge of firearms and the firing of live ammunition, and also those activities by other inexperienced participants and others.
Participant hereby acknowledges and agrees that the Participant has read this instrument and understands its terms and is executing this instrument voluntarily. Participant furthermore agrees that she/he has read, understands and will at all times abide by all range rules and procedures and any other rules and procedures stated by anyone associated with the AOSNI or the premises.
Participant expressly agrees that this instrument is intended to be as broad and inclusive as permitted by law, and that if any provision of this instrument is held invalid or otherwise unenforceable, the enforceability of the remaining provisions shall not be impaired thereby. No remedy, and each and every remedy shall be cumulative and shall be in addition to every other remedy now or hereafter existing at law or in equity or by statute or otherwise. BOTH PARTIES AGREE THAT ANY DISPUTE BETWEEN THEM REALTED TO THIS AGREEMENT OR OTHER SHALL BE TRIED BY A JUDGE AND THEY WAIVE THEIR RIGHT TO A TRIAL BY JURY. The election of anyone or more remedy hereunder by the AOSNI shall not constitute any waiver of AOSNI’s right to pursue other available remedies. This instrument binds Participant and her/his executors, personal representatives, administrators, assignees, heirs and next of kin.

IF PARTICIPANT IS A MINOR, PARENT OR LEGAL GURADIAN MUST ALSO SIGN BELOW:
I, the legal guardian of the Participant named above do execute this agreement personally, in my role as parent and/or legal guardian and on behalf of my child, each to the fullest extent allowed by law, and to the extent not allowed, I do further hereby agree to indemnify AOSNI from any loss or liability as if I were Participant.
